The state revokes, suspends licenses, certificates, registrations of health care providers

OLYMPIA - The Ministry of Health of Washington has revoked or suspended the licenses, certificates or registrations of the following health care providers in our country. The department also immediately suspended the credentials of people banned from practicing in other states.

The Department of Quality Assurance in Health Systems works with boards, commissions, and advisory committees to set licensing standards for more than 80 health professions (e.g., dentists, nurses, counselors). Information on disciplinary action taken against physicians and physician assistants can be found on the Washington Medical Commission (VMC) website. Information on health care providers can be found on the agency’s website. Click “Search for a health care provider’s license” in the “How-to?” Section of the Department of Health’s homepage (doh.va.gov). The site contains information on the status of licenses of health care providers, the expiration date and renewal of their credentials, disciplinary measures and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think that the health worker acted unprofessionally are also encouraged to call and report their complaint.

Chelan County

In March 2022, the Hearing and Speech Board suspended the speech therapist Traci A. Bair Hutchins (LL00003176). Hutchins noted the treatment of patients, but never provided services while working in a rehabilitation facility.

Clallam County

In March 2022, the Secretary of Health revoked the license for home care assistance John Stephen Gale (HM60604379). In 2021, Gail was convicted of lethal assault and harassment with death threats at the Clalam County High Court.

Kitsap County

In April 2022, the Secretary of Health suspended the license of a trainee for substance use disorder Douglas Paul Brown (CO60956432). Brown abused controlled substances.

Pierce County

In April 2022, the Nurses Commission suspended the license of a registered nurse Teresa Elizabeth Kit (RN00114854) after withdrawing from the substance use monitoring program in which she agreed to participate in order to avoid potential discipline.

Snohomish County

In April 2022, the Secretary of Health suspended the license for a housekeeper Austin Riley McLaughlin (HM60768975) after being convicted of assault in connection with domestic violence in Mersville Municipal Court.

Spokane County

In April 2022, the Secretary of Health suspended the license of a registered medical assistant David I. Kolesnikov (NA60246330). Kolesnikov did not give the patient medication on schedule and removed personal belongings from another patient’s room without permission.

In April 2022, the Secretary of Health rejected a request to reactivate an authorized medical assistant for Muriel R. Snakeskin (NC60086077) and suspended her license. Snakeskin did not respect the agreement from 2015, which required her to complete additional education and pay a fine.

Montana: In March 2022, the Nursing Commission suspended the license of a registered nurse Kelly Morrow Koch (RN00087640) after the Montana State Board of Nursing revoked Koch’s registered license as a nurse for failing to use an appropriate assessment of safe nursing practice and improperly delegated nursing care.
